Doctor Disability Referral Program Terms of Service

The DoctorDisability Refer-a-Friend Program (the "Program") enables participants living in eligible states to earn Rewards by recommending DoctorDisability to friends or family who are potentially interested in purchasing insurance. To participate, you must agree to these DoctorDisability Refer-a-Friend Program Terms & Conditions (the "T&C"), as well as DoctorDisability’ Terms of Use and Privacy Policy.

Definitions

As used in these Terms and Conditions of Use, the following terms have the following meanings:

Qualified Disability Referral - The referral of a physician or dentist who submits a quote request for disability insurance with DoctorDisability and reviews those quotes over the phone with a DoctorDisability agent.

Reward - A $50.00 e-gift card sent via email within 5 business days of DoctorDisability receiving a Qualified Disability Referral.

Overview of the Program

DoctorDisability will provide you with a unique Refer-a-Friend Program Link (your “Link”) so that DoctorDisability can associate you with individuals you refer.

By taking part in the Program, you agree not to:

Eligibility

You must have created an account with DoctorDisability to participate.

You may only participate in the Program if you are 24 years old or older and a legal resident of a Participating State.

Directors, officers, and employees of DoctorDisability and its affiliates (including affiliated companies, agents, representatives, licensees, and advertising or promotional agencies) and their immediate family members are ineligible to participate in the Program.

The Program is subject to all applicable federal, state and local laws and regulations and is void where prohibited or restricted by law.

Qualified Referral

Qualified Disability Referrals are limited to individuals who are not current DoctorDisability customers, are 24 years old or older, are a physician or dentist, and are legal residents of the United States.

Any fraudulent referrals (for example, using a different phone number or email address for the same person or intentional submission of false information) will be voided.

A referred individual may only use one referral link. Only the owner of the referral link used by the referred individual to request disability quotes will receive a Reward.

This Program may not be combined with other referral programs or incentives.

Participating States

This Program is only available to participants in the following states: Alabama; Arizona; Arkansas; California; Colorado; Connecticut; District of Columbia; Florida; Georgia; Hawaii; Idaho; Illinois; Indiana; Kansas, Kentucky; Maine; Maryland; Minnesota; Missouri; Montana; Nebraska; Nevada; New Jersey; New York; North Carolina; North Dakota; Ohio; Oklahoma; Oregon; Rhode Island; South Carolina; South Dakota; Texas; Utah; Vermont; Washington, Wisconsin; and Wyoming.

Rewards

In a twelve-month period, individuals will not be eligible to receive more than seven Rewards for Disability referrals ($350). No purchase by you or the referred individual is necessary to participate in the Program.

You will be notified via email when you have earned a Reward and will be required to claim your Reward via the link in the email. Rewards are administered by a third-party. You consent with sharing your personal information with that third-party for the purposes of administering the Program.

Rewards will be in the form of an eGift Cards. Restrictions, conditions, and limitations may apply.

DoctorDisability will not replace any lost or stolen eGift Cards.

DoctorDisability may combine multiple Rewards as a single eGift Card at its discretion.

Taxes and Fees

You are responsible for all applicable taxes and fees resulting from any Reward you receive under the Program.

Release of Liability

By participating in the Program, you release DoctorDisability and its agents from all liability, including, without limitation, all liability with respect to any and all Rewards.

Severability

If any provision in these T&C’s are held to be invalid, void, or unenforceable, such provision (or the part of it that is making it invalid, void, or unenforceable) will be struck and will not affect the validity and enforceability of the remaining provisions.

Termination and Change

DoctorDisability may suspend or terminate the Program or a user's ability to participate in the Program at any time for any reason without prior notice.

DoctorDisability reserves the right to deny any Rewards if it notices any activity that it believes is abusive, fraudulent, or contrary to the spirit of the Refer-a-Friend program (including where it appears in DoctorDisability’ sole determination that the Refer-a-Friend program is being used as a money-making scheme or insurance applications are being submitted solely to generate rewards).

DoctorDisability reserves the right to review and investigate all referral activities and to modify referrals as it deems fair and appropriate.

Updates to these T&C

DoctorDisability can update these T&C, including but not limited to the eligibility requirements, Participating States, and Reward amounts, at any time for any reason without prior notice. Such modification is effective upon posting of the modified T&C on the DoctorDisability website. Continued participation in the Program after any modification shall constitute consent to such modification.
